PROGRAM OVERVIEW
Throughcare aims to support the successful reintegration of offenders returning to the community at the end of their head sentence—i.e., prisoners released with/without parole. The Prison to Work report described the concept of ‘throughcare’ in the following terms:

Prisoner throughcare projects provide comprehensive case management for a prisoner in the lead up to their release from prison and throughout their transition to life outside. Projects aim to make sure prisoners receive the services they need for successful rehabilitation into the community… Good throughcare ‘starts in custody well before walking out of the prison gate’, and provides hands on, intensive support, especially at the moment of release.

POSITION OVERVIEW
As part of Yeddung Mura’s partnership with ACT Corrective Services’ throughcare program, the Outreach worker will be responsible for applying case management practices to clients who have been released from the Alexander Maconochie Centre.
The Outreach worker will be required to establish links with the local Aboriginal community in the region to encourage and support clients with reintegration.
Under limited direction, the role will involve (but not limited to):

Supporting clients to successfully integrate back into the community
Community Liaison
Practical Assistance
Feedback on Problems
Conducting risk assessments /preparing case plans /providing interventions/
The outreach worker must demonstrate exceptional client engagement and case management skills, including sound interviewing skills.

WHAT YOU WILL DO

The Peer Support Worker works collaboratively and aims to assist in improving the wellbeing of clients in the program by;
supporting clients and their families,
o developing and maintaining relationships with relevant service providers,
o providing peer support to clients and
o supporting evaluation of the Program.
Community Liaison – establish links with the local Aboriginal and Torres Strait Islander communities to promote Yeddung Mura objective of reintegration of the clients.
Administration and Support – assist Yeddung Mura to identify barriers to reintegration services for Aboriginal and Torres Strait Islander people.
Feedback on problems – provide feedback to the employer and Yeddung Mura regarding barriers to services for Aboriginal and Torres Strait Islander people and, in conjunction with the Yeddung Mura, work to implement solutions.
Distribute information and resources to the Aboriginal and Torres Strait Islander communities. Provide safe and timely transport services for clients to appointments.
Property Management as required
Actively establish and maintain relationships with internal and external stakeholders to ensure high quality service
Undertake administrative duties as required in accordance with program and organizational guidelines
Contribute to developing a culturally safe workplace for Aboriginal employees and clients
Participate in organization-wide, site based and team meetings, collaborative planning activities and other meetings or activities relevant to position
Contribute to the continuous improvement of systems and processes ensuring services meet professional and industry standards

WHAT YOU WILL REQUIRE
Technical expertise

Lived experience of being through the system
Practical knowledge and understanding of issues of detainees and their families
An approachable and accepting manner and attitude
Qualification or relevant experience

Certificate IV in Mental Health or related discipline or willingness to work towards.

  13. Provide individualized risk assessments, case plans, service coordination, and interventions to reduce the risk of clients re-offending.
  14. Demonstrate sound computer skills to maintain client records using case management software and produce reports and other documents using word processing systems.
  15. Foster good working relationships with other team members and encourage a high standard of practice by providing guidance, mentoring and support. 

WHAT YOU WILL REQUIRE 

Technical expertise 

  1. Demonstrated experience and knowledge of best practice case management frameworks, including the provision of needs assessment in human services context.
  2. Ability to implement theoretical concepts to reduce risk and maximize individual outcomes. 

Qualification or relevant experience 

  1. Certificate IV in Mental Health or related discipline or willingness to work towards.
  2. Mental health First Aid Certificate or willingness to work towards

The Benefits of Mentoring  

Nearly 1002 adults are released from ACT prisons each year. Almost all of them will eventually be released—and unless something changes, more than half will not be successful in re-entering their communities and will return to prison. It is estimated over $0.85 million dollars will be saved per year for 5 prisoners and $1.3 million dollars will be saved for 8 inmates avoiding recidivism. (The costing estimates are based on data provided by ABC news 30 January 2014 with an estimated cost of $465 per prisoner per day) 

Mentoring program is an attempt to reduce recidivism and to help people avoid re-offending by promoting smooth transition back into the community. 

Both Mentors and Mentees will be benefited by this program

 The program has been targeted to assist the participant in obtaining meaningful employment, raising his education level, improving his attitude towards himself and others; and increasing his ability to function and participate in his community in a socially acceptable manner. 

Where a prisoner is without strong connections, they are prone to stress and reinforcing feelings of hopelessness and worthlessness.  

Yeddung Mura is committed to the concept of a second chance for offenders and will work with stakeholders to seek the provision of such. 

Mentor Roles and Responsibilities 

Role 

  • Participate in a positive mentoring experience that supports the Mentee(s) towards good pathways.  
  • Contribute experience and wisdom on life issues by increasing the Mentee(s) practical knowledge 
  • Establish a good relationship with assigned Mentee(s) 
  • Build a relationship of mutual trust and respect  
  • Provide support to build confidence and expand life skills by providing an opportunity for deeper thinking, reflection and guidance 
  • Facilitate group mentoring and individual mentoring sessions in accordance with the needs of the group 
  • Provide honest, authentic and constructive feedback 

Responsibilities 

  • Make a necessary 12 months commitment to the program 
  • Attend the Yeddung Mura Mentoring Orientation Training Session 
  • Meet allocated mentee(s) for at least one hour every month for 12 months 
  • Link mentee(s) with any broad opportunities that may arise during the twelve-month program 
  • Provide general feedback about mentees’ progress to achieve the objective of the program 
  • Provide information necessary for Yeddung Mura evaluation of the program. 

Requirements 

  • A desire to be Yeddung Mura Mentor and be willing to support a mentee 
  • First aid mental health certificate 
  • Working with Vulnerable people card 
  • Understanding of Yeddung Mura policies and procedures 

Time Commitment 

  • At least one hour every month for 12 months  
  • Determine and self-regulate any extra time required for one-on-one mentoring between Mentor and Mentee(s) 

Meeting Place 

  • The Mentoring Sessions will be held at a place determined by the mentor/mentee.

 By joining the team you will get the opportunity to:

A minimum commitment of just four hours a week is required to volunteer.

No experience is necessary and all training is provided.
