The Case Management Framework is based on the belief that success for reintegrating into the community depends on a healthy mind and body.
The Yeddung Mura Throughcare Program works in collaboration with ACT Corrective Services Throughcare Unit to provide individualised and intensive case-managed and trauma-informed support to Aboriginal and Torres Strait Islander men and women exiting the Alexander Maconochie Centre following a custodial sentence.

Prison Case Mangement

Short-Term Plan
If a client does not receive Yeddung Mura (YM) services under the Pre-Prison Program, then a short-term plan is required. A case management plan is created after a prisoner has been recommended by AMC to receive YM services. YM interviews the detainee. Once there is an agreement to provide and receive services tasks are identified that will be actioned prior to the long-term plan commencing.

Long-Term Plan
This plan will includes services to be provided up until the release day. At some point during this period a release day plan is also created

Release Day Plan
At some point during the long-term case management period a release day plan is also created.

Community Engagement Plan
During the long-term case management period a reintegration plan is put in place. The benefit of this is that the detainee is aware what occurs after release day unto the 12th week of their release.

Post Release Case Mangement

Community Engagement
After the release day the community engagement plan commences. During this period there maybe a slow integration to the family. This is particularly so where there is a history of addictive behaviour, and domestic violence. It is common during this phase that the clients needs a lot of grief support. This a the most busiest of the phases as clients have needs with obtaing identification and estasblishing finances. Obtaining identificaiton brings a quick win to the emotional support of the client. During this phase there is daily contact with the client to best prepare him for seeking employment. Develop a Commujnty Spport Plan

Community Support Plan
During the 12 weeks of the Community Engagement period, a Community Support Plan is developed. The gold is for the client to move to unassisted support at the end of the 12 weeks whilst on a life-long path of continous learning and growth. During this period those with addictive behaviours, at risk of domestic abuse or any risk to reoffend benefit from an individual case plan.
